Debenham Group Practice are looking for a care navigator to join the team on a part-time basis working Wednesday, Thursday and Fridays each week. Shifts fall within a working window of approximately 07:30 to 18:30 across those days. We welcome applications from individuals who can cover all three days as well as those who are available for part of the pattern, so please do apply if mid-to-end of week working suits your availability. Depending on recruitment, hours may be split across more than one person.
This is a patient-facing role, at the heart of how the practice runs day-to-day, and we are looking for someone who will bring a calm, friendly and organised approach to supporting our patients and colleagues. You will work across our three sites, as part of a well-established, friendly team.
Care navigators are often the first voice or face a patient encounters. Whether patients contact us by phone, walk through the door, or submit a request online, you will help make sure they get the right information and reach the right place.
We use askmyGP as our primary workflow tool for clinical requests. Rather than triaging patients themselves, Care Navigators help ensure that requests are accurately recorded so that the duty GP can review and act on them promptly. You will also take turns supporting the duty GP as part of the wider Care Navigation team, and you will help signpost patients to other local services where that is the most appropriate response.
The role also covers the full range of day-to-day practice administration from reception duties and managing records, to supporting clinical and dispensary colleagues and keeping the practice running smoothly.
You will provide a courteous, efficient and helpful service to patients and colleagues, support clinical and administrative teams with a range of tasks, and help ensure the smooth running of the practice across all sites.
