Social Prescriber

Sudbury & West Suffolk Rural (S&WSR) Primary Care Network has an exciting opportunity for a social prescriber to join our practice team on a part-time basis, for 16 hours per week. The hours will be shared across Guildhall Surgery in Clare, and Siam Surgery in Sudbury.

Social prescribers are responsible for providing non-clinical support by linking patients to community and voluntary services across their area, adopting a holistic approach to supporting patients to improve their health and wellbeing. We are looking for someone to join our team in order to provide this service across S&WSR PCN at, Clare Guildhall, and Siam Sudbury Surgeries.

The successful candidate will be responsible for providing one-to-one support, identifying issues that impact on patients' health and wellbeing, developing personalised care and supporting plans to address their needs, making onward referrals to community and voluntary sector services, and recording patient outcomes.
