Practice Nurse

The Colne Practice, located in Rickmansworth, is seeking an experienced practice nurse to join their busy practice, preferably with a respiratory interest (asthma, COPD, spirometry). This position is offered on a part-time basis, for 2 days per week, working a Thursday and a Friday. GP practice experience is essential.

Working as a member of the multidisciplinary team, the successful individual will ensure nursing services are delivered effectively to the entitled patient population and implement processes for the management of patients with long-term conditions.

Main responsibilities will include:

• Asthma, COPD, spirometry
• Diagnose and manage acute and chronic conditions
• Provide wound care (ulcer/Doppler, etc.) to patients
• Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice
• Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
• Develop, implement and embed health promotion and wellbeing programmes
• Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
• Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients’ health
