PCN Patient Care Co-ordinator

The Deben Health Group PCN is looking for an enthusiastic, organised and compassionate Care Coordinator to join our award-winning, friendly multidisciplinary team. This position is offered on a full or part-time basis working up to 37.5 hours per week.

Supporting approximately 38,500 patients across Wickham Market Medical Centre, The Peninsula Practice, Little St John’s Street Surgery and Framfield House Surgery, you will help people navigate services and access timely, coordinated support.

This varied role combines administrative care coordination with social prescribing support.

You will carry out the following duties:

• Liaise with patients, carers, practice teams, community services, care homes and voluntary organisations
• Coordinate referrals and follow-up actions
• Support multidisciplinary meetings
• Connect people with services that can improve their health and wellbeing
